Visiting Privileges Process

To apply for Visiting Privileges at UCSF Medical Center, please follow these steps.
You will need Adobe Acrobat Reader, to open and print the forms.

Step 1: Download Visiting Privileges Application Form  (Editable Form - Type in the information and print for signature)

Step 2: Download the Privileges form

Step 3: Review/Download Bylaws, Rules and Regulations

Step 4: Submit the complete application and privileges to the Department you are applying for privileges:
               Department Chair and Address

Step 5: Department chair reviews the application, and after approving, submits to the Medical Staff Office for processing.
