Allied Health Membership Process

To apply for Allied Health Membership at UCSF Medical Center, please follow these steps.
You will need Adobe Acrobat Reader, to open and print the forms.

Step 1: Download the Allied Health Application

Step 2: Download the AHP Privilege Form (including, if applicble, standardized procedures and medication formulary)

Step 3A: Submit the completed application and privileges to your Supervising Physician and Department Chair for review.  Department Chair and Address

Step 3B: The Supervising Physician and Department Chair should sign the "Department Chair’s Cover Letter"

Step 4: Submit to the Medical Staff Office for processing.
